Blood sugar management is central to the nutritional care of millions of people — not just those with diabetes, but anyone managing energy levels, weight, cardiovascular risk, or hormonal health. Yet one of the most commonly cited tools for this purpose — the Glycemic Index — is frequently misunderstood and misapplied in practice.
What Is the Glycemic Index?
The Glycemic Index (GI) ranks carbohydrate-containing foods on a scale of 0–100 based on how rapidly they raise blood glucose compared to pure glucose. High-GI foods (≥70) cause rapid glucose spikes; low-GI foods (≤55) produce a more gradual rise.
The problem: GI is measured under standardized laboratory conditions using a 50g carbohydrate portion of the test food. This doesn't reflect how foods are actually eaten — in mixed meals, in varying quantities, with different preparation methods.
Enter Glycemic Load
Glycemic Load (GL) corrects for this by accounting for the actual amount of carbohydrate in a typical serving:
GL = (GI × grams of available carbohydrate per serving) ÷ 100
The classic illustration: watermelon has a high GI of 72 — but a standard 120g serving contains only about 6g of available carbohydrate, giving it a GL of just 4. On the GI scale alone, watermelon looks like a food to avoid for blood sugar management. With GL, it's clearly fine.

Practical Strategies Beyond GI and GL
The 2015 International Consensus Summit (Augustin et al.) reinforced that while GI and GL are useful tools, overall dietary patterns and food combination strategies also significantly impact glycemic response:
- Food order matters: Eating vegetables and protein before carbohydrates in a meal can reduce post-meal glucose by up to 30%
- Cooling and reheating: Cooking and cooling starchy foods (pasta, potatoes, rice) increases resistant starch content, lowering glycemic impact
- Vinegar and acid: Acetic acid (from vinegar, lemon juice) slows gastric emptying and blunts glucose spikes
- Fibre pairing: Combining refined carbohydrates with fibre-rich vegetables significantly reduces their glycemic impact
- Protein and fat: Both slow digestion and reduce the glycemic response to carbohydrates
Clinical Application
For dietitians working with clients managing type 2 diabetes, insulin resistance, or PCOS, GL-based meal planning is a more nuanced and practical framework than GI alone. It allows clients to understand not just which foods affect blood sugar but how much and — critically — how to modify that impact through food combinations and preparation methods.
